Question 1 One of the questions missed is asking whether Racheal had any issues with heat or cold intolerance, loss of appetite, increased thirst or sweating. This was an important question to ask as Goolsby and Grubbs (2018) indicate that simple changes such as loss of appetite, weight loss, and fatigue may help diagnose certain malignancies and potential endocrine system problems presenting with nonspecific complaints. A second question that was missed is whether Racheal had had movable breast lumps or discharge from her nipples. Sometimes, breast discharge and some movable lumps are an indication of some forms of breast cancer, and it is important to ask such a question to better understand the underlying condition.Question 2 One of the errors made in the physical examination was not palpating the lymph nodes, the neck, and the abdomen. This has the impact of missing some important information about Racheal’s condition. A second error was that I was thorough skin, nails, and hair examination collecting information that was not needed as was indicated by the expert as the goal was examining the breast.Question 3 One of the key findings from the physical examination is the two palpable lumps on Racheal’s right breast indicating possible breast cancer. Question 4 The development of cancer is gradual and is mostly influenced by genetic and environmental factors. One of these factors is alcohol consumption which according to Bichley (2016) increases an individual’s risk of developing cancer by between 1.1 to 2.0 times. History of alcohol consumption is an identification problem that was not chosen for Racheal. Although she quit taking alcohol six years ago, her time-consuming alcohol might have acted as a risk factor for cancer development.Question 5 One of the incorrect differential diagnosis was breast lipoma.
